Approach to Hair Loss for New Nurse Practitioners
Listen

Show notes:
Patients coming into primary care with a chief complaint of hair loss can be so stressful as a new nurse practitioner.
First, let’s normalize this. It’s a broad differential and can be tricky to parse out. It can also feel really stressful and emotional for your patient presenting for care, which adds even more pressure.
Diagnosing Hair Loss in Primary Care
In this week’s video, I’m covering how to approach hair loss in primary care, the differential diagnoses, history, physical exam and testing highlights for new nurse practitioners in primary care.

Hey there, welcome to the Real World NP podcast.
2
00:00:13.140 --> 00:00:20.400
I'm Liz Rohr, family nurse practitioner, educator, and founder of Real World NP, an educational
3
00:00:20.400 --> 00:00:23.140
company for nurse practitioners in primary care.
4
00:00:24.600 --> 00:00:29.420
I'm on a mission to equip and guide new nurse practitioners so that they can feel
5
00:00:29.420 --> 00:00:33.020
confident, capable, and take the best care of their patients.
6
00:00:33.520 --> 00:00:38.280
If you're looking for clinical pearls and practice tips without the fluff, you're in
7
00:00:38.280 --> 00:00:39.080
the right place.
8

11
00:01:02.490 --> 00:01:05.550
Hey there, it's Liz Rohr from Real World NP.
12
00:01:06.050 --> 00:01:12.070
You are watching the Real World NP YouTube channel where I share weekly episodes to help
13
00:01:12.070 --> 00:01:17.410
save you time, learn faster, and help you take the best care of your patients.
14
00:01:17.970 --> 00:01:23.130
In this week's episode, we are going to be talking about hair loss as a chief complaint
15
00:01:23.130 --> 00:01:24.090
in primary care.
16
00:01:24.570 --> 00:01:26.230
So this is a tricky one.
17
00:01:26.230 --> 00:01:30.310
I just want to normalize that because when it comes to this chief complaint,
18
00:01:30.570 --> 00:01:32.730
the differentials are really actually broad.
19
00:01:32.730 --> 00:01:38.230
In this episode, I'm going to focus on breaking it down with a kind of quick and dirty approach
20
00:01:38.230 --> 00:01:44.250
in primary care and what are the red flags of what not to miss and things to think
21
00:01:44.250 --> 00:01:45.730
about as an initial approach.
22
00:01:46.250 --> 00:01:49.110
And in this episode, I'm actually going to work backwards.
23
00:01:49.150 --> 00:01:54.490
So I usually start with history questions, exam, things like that, and then I get to
24
00:01:54.490 --> 00:01:57.990
differentials and plan, but I'm going to start actually with differentials.
25
00:01:58.570 --> 00:02:04.750
And I've said this before, but as I've become more experienced as a nurse practitioner,
26
00:02:05.250 --> 00:02:09.770
when I encounter a chief complaint, I get more of the differentials in the front of
27
00:02:09.770 --> 00:02:18.410
my mind such that when I ask the history HPI questions, I can ask those history questions
28
00:02:18.410 --> 00:02:20.630
specific to the differentials.
29
00:02:20.950 --> 00:02:23.010
So we're going to practice it in this video.
30
00:02:23.690 --> 00:02:24.190
Who knows?
31
00:02:24.250 --> 00:02:27.530
I think it'll make more sense for this particular topic because it will really
32
00:02:27.530 --> 00:02:30.330
frame what it is you're asking for, what you're looking for.
33
00:02:31.250 --> 00:02:33.150
So let's start with a couple of differentials.
34
00:02:33.510 --> 00:02:37.970
So like I said, hair loss has a very broad differential.
35
00:02:38.410 --> 00:02:40.910
And so these are the main ones I want you to keep in mind.
36
00:02:41.270 --> 00:02:44.510
So on the one hand, there are systemic illnesses.
37
00:02:45.090 --> 00:02:46.330
So I'll talk about that in a second.
38
00:02:46.490 --> 00:02:52.970
And then the other thing is like, is this a scalp or hair disorder compared to the
39
00:02:52.970 --> 00:02:54.270
systemic illnesses, right?
40
00:02:54.450 --> 00:02:55.690
So let's talk about some of those.
41
00:02:55.690 --> 00:02:59.410
So what are some systemic illnesses we need to think about?
42
00:02:59.830 --> 00:03:03.490
One is hyper or hypothyroidism can cause hair loss.
43
00:03:04.090 --> 00:03:09.250
Another one is iron deficiency anemia or B12 deficiency anemia.
44
00:03:10.430 --> 00:03:16.890
Another one is actually in a kind of like a broader category.
45
00:03:16.910 --> 00:03:21.290
So there's something called telogen effluvium, which is a really fancy word
46
00:03:21.290 --> 00:03:24.310
and make you feel real fancy when you use that in your notes or when you're
47
00:03:24.310 --> 00:03:25.370
discussing with colleagues.
48
00:03:26.070 --> 00:03:31.830
But basically what it's referring to is that in hair, there are three general
49
00:03:31.830 --> 00:03:38.810
phases, antigen, captogen and telogen, meaning growth, development through rest.
50
00:03:39.330 --> 00:03:43.910
And about at any one time on the average person's head, 90 percent of them
51
00:03:43.910 --> 00:03:46.350
are in the first two phases, antigen, captogen.
52
00:03:46.490 --> 00:03:47.410
I hope I'm saying them right.
53
00:03:47.490 --> 00:03:49.070
I'm really good at mispronouncing things.
54
00:03:50.130 --> 00:03:52.270
And then telogen is that resting phase.
55
00:03:52.270 --> 00:03:56.890
So about 10 percent of hair is in that telogen phase, that resting phase.
56
00:03:56.890 --> 00:03:58.490
And that's when it falls out, right?
57
00:03:58.490 --> 00:04:02.410
Because there's continual, continual growth, shedding, all that stuff.
58
00:04:03.010 --> 00:04:05.870
There are certain states that lead to telogen effluvium.
59
00:04:06.370 --> 00:04:08.050
It is it is a whole can of worms.
60
00:04:08.710 --> 00:04:12.170
But long and short of it for the purposes of this video
61
00:04:12.790 --> 00:04:18.570
and your first approach to hair loss and primary care is major, major
62
00:04:18.570 --> 00:04:23.950
changes in the body can lead to kind of like a general amount of hair loss
63
00:04:23.950 --> 00:04:27.990
that is almost like when you pull on your hair, a whole bunch comes out at once.
64
00:04:27.990 --> 00:04:29.010
It's not like patchy.
65
00:04:29.110 --> 00:04:31.290
It's not specific bald spots, things like that.
66
00:04:32.270 --> 00:04:33.150
So that can come.
67
00:04:33.330 --> 00:04:34.970
I believe that can come with this.
68
00:04:35.070 --> 00:04:39.970
That is how it's shown in hyper or hypothyroidism and iron deficiency as well.
69
00:04:40.130 --> 00:04:44.090
But the other thing that can cause that is things like major stress,
70
00:04:44.390 --> 00:04:48.110
whether it's emotional stress, physiologic stress, chronic illness.
71
00:04:48.570 --> 00:04:51.530
Or acute illnesses, hospitalizations.
72
00:04:52.610 --> 00:04:54.450
And the other thing is pregnancy.
73
00:04:54.750 --> 00:04:57.490
So postpartum, it's actually a specific name called
74
00:04:58.830 --> 00:05:00.730
Telogen Gravidarum, I believe.
75
00:05:01.070 --> 00:05:02.470
Anyway, that's just being real fancy.
76
00:05:02.950 --> 00:05:06.090
But that happens predictably after childbirth.
77
00:05:06.410 --> 00:05:10.770
So there are certain states that can have this telogen effluvium state,
78
00:05:10.770 --> 00:05:14.870
and it tends to happen two to three months after the inciting incident.
79
00:05:15.330 --> 00:05:18.330
Kind of two, one or two other things that can cause it.
80
00:05:18.450 --> 00:05:21.410
That's pretty important to remember, because I see this all the time,
81
00:05:22.330 --> 00:05:26.550
is sudden decreases in caloric intake or vitamin intake.
82
00:05:26.950 --> 00:05:30.370
So, for example, when somebody goes on a very strict diet
83
00:05:30.370 --> 00:05:35.030
or caloric restriction, anorexia nervosa, perhaps after bariatric surgery,
84
00:05:35.610 --> 00:05:37.770
those things and again, having a surgery as well,
85
00:05:38.190 --> 00:05:40.930
that decrease in nutrition can trigger that to happen.
86
00:05:41.310 --> 00:05:44.130
And then that can either get better over time
87
00:05:44.130 --> 00:05:48.170
or when the calories are reinstated, then less of an issue.
88
00:05:49.230 --> 00:05:53.190
One other thing can be there's some correlation with vitamin D deficiency
89
00:05:53.190 --> 00:05:57.510
and hair loss, that general kind of like a general hair loss, right?
90
00:05:57.510 --> 00:05:58.530
Not like specific patches.
91
00:05:59.110 --> 00:06:01.510
So that would be one to consider as well.
92
00:06:02.070 --> 00:06:05.450
So the other kind of those are kind of the more systemic related issues
93
00:06:05.450 --> 00:06:07.770
that we want to keep front of mind with hair loss.
94
00:06:08.110 --> 00:06:11.070
The other ones are scalp specific disorders
95
00:06:11.070 --> 00:06:14.670
and or hair shaft follicle disorders.
96
00:06:15.030 --> 00:06:17.390
And like I said, that's a big big chunk of stuff.
97
00:06:17.390 --> 00:06:18.270
So we'll keep it at that.
98
00:06:18.350 --> 00:06:21.610
But the main kind of skin ones we want to think about in primary care.
99
00:06:22.410 --> 00:06:24.570
Do we have any seborrheic dermatitis?
100
00:06:24.850 --> 00:06:27.690
Do we have any tinea capitis?
101
00:06:28.190 --> 00:06:31.050
And then one other kind of very
102
00:06:31.050 --> 00:06:35.010
on the more common side one is traction alopecia,
103
00:06:35.330 --> 00:06:38.950
which has to do with pulling on the hair from various hairstyles
104
00:06:38.950 --> 00:06:41.530
that can lead to spots of thinning hair.
105
00:06:42.890 --> 00:06:43.790
So you want to ask,
106
00:06:44.110 --> 00:06:48.250
this is leading into the next section of history and exam and pearls of practice.
107
00:06:48.570 --> 00:06:52.170
But you, of course, want to ask about how patients take care of their hair,
108
00:06:52.170 --> 00:06:53.570
what they do for it.
109
00:06:53.910 --> 00:06:57.490
Do they have, you know, tighter hair style, like things like that.
110
00:06:57.730 --> 00:07:02.550
Other things as it relates to kind of like hair specific or scalp specific,
111
00:07:02.830 --> 00:07:08.430
you want to think about, is there any type of like, what is the pattern?
112
00:07:08.430 --> 00:07:11.010
OK, so this is getting to the next section of like history and exam.
113
00:07:11.430 --> 00:07:12.370
So let's just jump in.
114
00:07:12.430 --> 00:07:16.550
So when we're talking about history, we want to ask whenever there's
115
00:07:16.550 --> 00:07:18.610
a chief complaint, regardless of what it is,
116
00:07:18.750 --> 00:07:22.490
and especially if you're not comfortable with the full differential diagnosis,
117
00:07:22.750 --> 00:07:26.550
you want to go back to your acronyms of old cart or whichever one you use.
118
00:07:26.990 --> 00:07:28.690
PQRST, I think some other people use.
119
00:07:28.990 --> 00:07:31.810
But what are the questions that we can ask to get a full history,
120
00:07:31.850 --> 00:07:33.670
regardless of the differentials, right?
121
00:07:34.410 --> 00:07:37.270
Specific to hair loss, though, we want to ask about duration.
122
00:07:37.270 --> 00:07:39.290
How long has this been going on for?
123
00:07:39.390 --> 00:07:40.950
Did it just start? Is it progressing?
124
00:07:41.110 --> 00:07:42.950
Is it getting worse, better or the same?
125
00:07:43.090 --> 00:07:44.550
Any sort of major stressors?
126
00:07:44.610 --> 00:07:47.190
Again, going back to the differential, I don't want to know.
127
00:07:47.250 --> 00:07:49.590
Is there a risk of telogen effluvium? Right.
128
00:07:49.590 --> 00:07:50.410
Are there patches?
129
00:07:50.650 --> 00:07:53.110
Is it specific areas on their scalp?
130
00:07:53.510 --> 00:07:54.710
Is there a pattern?
131
00:07:54.930 --> 00:07:57.550
And if you're listening, instead of watching, I'm pointing to my hairline.
132
00:07:57.610 --> 00:08:01.990
But is there a receding of a hairline or is there loss by the crown
133
00:08:01.990 --> 00:08:03.190
or in the part?
134
00:08:03.810 --> 00:08:06.010
And you want to ask about, you know,
135
00:08:06.050 --> 00:08:08.310
do they have any physical symptoms associated with it?
136
00:08:08.310 --> 00:08:09.250
Is there any itching?
137
00:08:09.410 --> 00:08:11.430
Is there any pain?
138
00:08:11.570 --> 00:08:13.250
Is there any drainage? Things like that.
139
00:08:13.310 --> 00:08:15.490
And one other differential I forgot to mention
140
00:08:16.070 --> 00:08:21.450
related to the hair loss patterns is that people can have androgen excess.
141
00:08:21.910 --> 00:08:24.230
And whether or not for
142
00:08:24.730 --> 00:08:27.470
for some patients that might have PCOS, for example,
143
00:08:27.730 --> 00:08:31.090
excess androgens or other conditions can lead to increased hair loss.
144
00:08:31.210 --> 00:08:32.870
And then there's some genetic components.
145
00:08:32.870 --> 00:08:35.010
Of genetic related hair loss.
146
00:08:35.110 --> 00:08:37.870
So, again, just to recap and tie it back into those differentials,
147
00:08:37.909 --> 00:08:42.890
do they have any recent weight loss, any diets, any recent surgeries,
148
00:08:43.210 --> 00:08:44.210
major stressors?
149
00:08:44.230 --> 00:08:47.190
And of course, we always want to ask about medications that they take.
150
00:08:47.450 --> 00:08:49.650
Even if we're not familiar with all the medications
151
00:08:49.650 --> 00:08:52.690
that can contribute to hair loss, we want to have that full history
152
00:08:52.690 --> 00:08:54.590
so it can be part of our investigation.
153
00:08:54.890 --> 00:08:56.550
So, again, the next part is the exam.
154
00:08:56.630 --> 00:08:58.830
So even if you don't feel super comfortable with this,
155
00:08:59.130 --> 00:09:02.590
you can generally speaking examine and palpate the scalp.
156
00:09:02.590 --> 00:09:03.610
And the hair.
157
00:09:04.150 --> 00:09:08.470
So you want to see, again, point to if they can point you to where the issues are.
158
00:09:08.690 --> 00:09:11.650
You can determine is it patches versus general hair loss
159
00:09:11.650 --> 00:09:15.370
versus if there are rash, signs of dandruff, seborrheic
160
00:09:15.370 --> 00:09:17.890
dermatitis, tinea capitis, et cetera, et cetera.
161
00:09:18.030 --> 00:09:19.030
Again, huge differential.
162
00:09:19.150 --> 00:09:20.590
But just start there. Right.
163
00:09:20.650 --> 00:09:22.830
And use your clinical judgment to determine
164
00:09:22.830 --> 00:09:25.510
is there a risk for some sort of systemic illness?
165
00:09:25.590 --> 00:09:26.990
Is this local? Things like that.
166
00:09:27.030 --> 00:09:29.310
When it comes to diagnostics.
167
00:09:29.310 --> 00:09:34.510
So, again, I'm really focusing on the main triage related stuff in primary care.
168
00:09:34.590 --> 00:09:37.730
And then you're kind of deciding from there, do they need to see dermatology?
169
00:09:38.090 --> 00:09:40.170
So we're thinking about, again, those differentials.
170
00:09:40.270 --> 00:09:42.010
Do we want to assess for their TSH?
171
00:09:43.130 --> 00:09:44.870
Do they have signs of androgen excess?
172
00:09:44.970 --> 00:09:47.890
Do we want to test for that iron deficiency?
173
00:09:48.170 --> 00:09:52.490
Do we want to do a CBC ferritin iron studies B12?
174
00:09:52.590 --> 00:09:55.790
Right. Because, again, if if if you've been a part of the lab course,
175
00:09:55.790 --> 00:09:59.550
we talk about how you might not see on the CBC that there's anemia,
176
00:09:59.910 --> 00:10:03.250
but your ferritin store, your iron stores reflected in your ferritin
177
00:10:03.250 --> 00:10:06.490
can be coming very close to iron deficient.
178
00:10:06.490 --> 00:10:08.230
And you just haven't caught it on the CBC yet.
179
00:10:08.230 --> 00:10:11.350
So you want to think about do you want to do a ferritin again or a B12?
180
00:10:11.470 --> 00:10:14.470
And then one other kind of I didn't mention this in the differential,
181
00:10:15.050 --> 00:10:17.170
but syphilis can cause patchy hair loss.
182
00:10:17.870 --> 00:10:21.290
Epidemiologically, not a huge necessarily population risk,
183
00:10:21.290 --> 00:10:24.410
but something you want to consider based on their history and their exam.
184
00:10:24.410 --> 00:10:29.490
And so really, your end result and your treatment is dependent on those factors.
185
00:10:29.710 --> 00:10:31.730
Right. And like I said, there's so many things that it could be.
186
00:10:31.890 --> 00:10:35.430
But if there's any risk for seborrheic dermatitis or tinnia,
187
00:10:35.470 --> 00:10:38.330
you could trial four to six weeks of kineconazole.
188
00:10:38.370 --> 00:10:40.410
If it seems like it's something related to that,
189
00:10:40.710 --> 00:10:43.290
you can do their blood tests and see if there's any risk
190
00:10:43.290 --> 00:10:44.670
for any of those factors.
191
00:10:44.890 --> 00:10:49.330
And then if it really looks like something that is outside of that scope,
192
00:10:49.610 --> 00:10:51.830
that's when you get the assistance of dermatology.
193
00:10:51.830 --> 00:10:54.530
So hopefully this video is helpful.
