Liver Function Test Interpretation (LFTs) for Primary Care
A Practical Framework for Interpreting Abnormal Liver Tests in Primary Care
When You Open the Chart and See Abnormal LFTs… What Do You Do Next?
When I first started practicing, abnormal liver tests were one of the labs that slowed me down the most. I remember opening results that were only mildly elevated and still feeling unsure what to do with them. Instead of quickly making a plan, they would sit in my inbox while I looked things up or tried to piece together the right next step.
At the same time, I was caring for patients in an FQHC clinic with chronic liver disease, hepatitis, cirrhosis, and occasionally AST/ALT levels in the thousands. Those results felt much more urgent—and honestly, pretty intimidating.
Over time, once I understood the patterns and the reasoning behind the workup, these labs became much easier to manage. Most of the time, the next step is actually very straightforward when you have a clear framework. This course was designed to share that framework, so when abnormal liver tests show up in your inbox or during a visit, you have a structured way to evaluate them, make a plan, and move forward with confidence.
What you’ll learn:
-
How to interpret high TSH and diagnose hypothyroidism
-
How to evaluate low TSH and recognize patterns of hyperthyroidism
-
How to distinguish overt vs subclinical thyroid disease and decide when to monitor vs treat
-
How to evaluate elevated prolactin levels, including common causes and when referral is needed
Why This TSH and Prolactin Interpretation Course is Different
Many endocrine courses fall into one of three categories: highly academic physiology reviews, specialty endocrinology education designed from a specialist perspective, or hormone optimization and functional medicine training.
This course takes a different approach—instead of focusing primarily on theory or specialty-level management, it is designed around the real decisions primary care clinicians face when abnormal endocrine labs appear in clinic—what the result means, what the next step should be, and when referral is truly necessary. The goal is to provide a clear clinical reasoning framework that makes endocrine lab interpretation more practical, structured, and easier to apply in everyday practice.
What’s Included in This TSH and Prolactin Interpretation Course
This self-paced course includes short video lessons that walk through a clear framework for interpreting abnormal liver tests in primary care. You’ll learn how to recognize common lab patterns, understand the physiology behind those patterns, and apply a structured approach to evaluation and management in clinic.
The course also includes practice cases, downloadable When to Worry Cheat Sheets, and slides for quick reference, and an audio-only playback option through the course app, so you can review lessons while commuting or between clinic sessions.
This activity includes the opportunity to earn 1.75 hours of ANCC continuing education credit upon completion of the post-test and activity evaluation.

Each course is grounded in current evidence and practical frameworks that translate seamlessly into day-to-day care—no fluff, no filler.
Interpreting abnormal endocrine labs is a common challenge in primary care. Nurse practitioners frequently encounter abnormal thyroid tests, including elevated TSH levels suggesting hypothyroidism and suppressed TSH levels associated with hyperthyroidism. Understanding how to interpret thyroid function tests—including TSH, free T4, and T3—is essential for accurate diagnosis and appropriate management of thyroid disease.
This course provides a practical framework for evaluating abnormal thyroid labs in adults, including identifying overt versus subclinical hypothyroidism, evaluating low TSH levels, and determining when treatment with levothyroxine is appropriate. Clinicians will also learn how to recognize patterns of hyperthyroidism, when to initiate symptom management, and when referral to endocrinology is necessary.
In addition to thyroid disorders, the course reviews the evaluation of elevated prolactin levels in primary care. Learners will review common causes of hyperprolactinemia, including medication effects, hypothyroidism, chronic kidney disease, and pituitary adenomas. The course also covers appropriate indications for prolactin testing, interpretation of prolactin levels, and when pituitary imaging or endocrine referral is indicated.
Designed specifically for nurse practitioners and other primary care clinicians, this course focuses on real-world clinical decision making and practical diagnostic frameworks for endocrine lab interpretation.
