Thyroid Lab Interpretation & Management for Primary Care

Move from recognizing abnormal thyroid labs to confidently managing them

Interpreting Thyroid Labs in Primary Care

Most nurse practitioners don't struggle to recognize an abnormal TSH—they struggle with everything that comes after.

Should you repeat the labs or start treatment? Is this overt or subclinical disease? Which additional tests actually change management? If you decide to treat, where do you start? And what about the patient who's already taking levothyroxine but still has an abnormal TSH? When should you adjust therapy, troubleshoot persistent abnormalities, or refer to endocrinology?

These are the decisions that come up every day in clinic—and they're rarely answered by a single laboratory value alone. The right next step depends on the patient's symptoms, thyroid hormone levels, medications, age, medical history, and clinical context.

This course teaches a practical, evidence-based approach to evaluating and managing thyroid disorders in primary care. You'll learn how to determine the appropriate workup, confidently decide when to monitor, treat, or refer, initiate and adjust levothyroxine therapy, counsel patients on what to expect, troubleshoot common management challenges, and safely follow patients over time. As a bonus, you'll also learn a structured approach to evaluating elevated prolactin, including common causes, appropriate workup, and when pituitary imaging or endocrinology referral is indicated.

What you’ll learn:

  • Interpret abnormal thyroid labs with confidence

  • Evaluate and manage hypothyroidism in primary care

  • Work up thyrotoxicosis and know when to refer

  • Bonus: Evaluate elevated prolactin and pituitary red flags

Why This Thyroid Course is Different

This isn't just a course about interpreting thyroid labs—it's about managing the patient from the first abnormal TSH through long-term follow-up.

You'll learn how to move from an abnormal TSH to a confident management plan: choosing the right follow-up tests, deciding when treatment is indicated, initiating levothyroxine therapy, adjusting treatment for patients already taking thyroid medication, monitoring response over time, counseling patients, avoiding common pitfalls, and recognizing when endocrinology referral is appropriate.

Using real-world patient scenarios and case-based practice questions, you'll build a repeatable clinical framework you can apply to the next patient—not just memorize for a test.

Each course is grounded in current evidence and practical frameworks that translate seamlessly into day-to-day care—no fluff, no filler.

Interpreting abnormal endocrine labs is a common challenge in primary care. Nurse practitioners frequently encounter abnormal thyroid tests, including elevated TSH levels suggesting hypothyroidism and suppressed TSH levels associated with hyperthyroidism. Understanding how to interpret thyroid function tests—including TSH, free T4, and T3—is essential for accurate diagnosis and appropriate management of thyroid disease.

This course provides a practical framework for evaluating abnormal thyroid labs in adults, including identifying overt versus subclinical hypothyroidism, evaluating low TSH levels, and determining when treatment with levothyroxine is appropriate. Clinicians will also learn how to recognize patterns of hyperthyroidism, when to initiate symptom management, and when referral to endocrinology is necessary.

In addition to thyroid disorders, the course reviews the evaluation of elevated prolactin levels in primary care. Learners will review common causes of hyperprolactinemia, including medication effects, hypothyroidism, chronic kidney disease, and pituitary adenomas. The course also covers appropriate indications for prolactin testing, interpretation of prolactin levels, and when pituitary imaging or endocrine referral is indicated.

Designed specifically for nurse practitioners and other primary care clinicians, this course focuses on real-world clinical decision making and practical diagnostic frameworks for endocrine lab interpretation.
